Applicants must have completed the registration to continue with the online filing of their Captain's Report. To file a new report, click on the ATO Reporting System link from the ATO Website homepage and Login using your designated user name and password. Select Process Air Traffic Incident.

Click the Next button once all entries on the page are filled out. A prompt would appear whenever a field is required asking you to supply an entry. The system would proceed to the next page whenever all entries are correct.

Click on the Cancel button to cancel the application at any time and return to the ALS home page.

You may use N/A on text fields requiring entry if you don't have any information available. A prompt will appear whenever an error or a requirement on a field is needed with the cursor focusing on the said field. Please refer to Entering Data page for questions regarding entering information.

You may go back to the previous page at any time to make corrections by clicking on the < Back button.

The last page of the online filing process is the confirmation page where the Reference Number is issued by the system. You may print this page to save a copy of your filing. You will use the above number for all your transactions with your document including tracking and follow-ups.

Print Application Form - Applicants can print print the filled-up form after receiving the confirmation of a successful filing.
